They certainly have their own Sith Lords, but they probably have no connections to the Sith Lords that emerged from Revan's armada.

 

The "True Sith" waiting in the Unknown Regions is probably the remnant of the Old Sith Empire that was defeated in the Great Hyperspace War 1,000 before the game. They fled into the Unknown Regions to hide, rebuild and eventually reconquer the galaxy. Somehow, they manipulated the Mandalorians into attacking the Republic so as to weaken them ahead of their own planned invasion. Revan pushed the Mandalorians back to Malachor V where he eradicated them. There, Revan figured out what was going on, and decided the Republic, with its battered economy and weak Jedi Council, was not strong enough to face the True Sith. So, he embraced the Sith philosophy himself and converted his Jedi followers, fueling them with a disdain for Jedi and weakness. By doing this, he firmly severed their allegiance to the Jedi Council and made them more accepting of his plans of conquest (this is my reasoning as to why Kreia described it as a "sacrifice" rather than a fall).

Actually, the Sith Empire was always in the area known as the Unknown Regions, at least during this age. After the second great schism, the dark jedi, exiled from the republic by the jedi, fled to Korriban, where they founded their Sith empire around 6900 BBY, almost 3000 years before the events of the KotOR games. Almost 2000 years later they tried to conquer the republic (the Great Hyperspace War), which failed and resulted in the fall of their empire. What happened to it after that is still uncertain.

 

Somehow, they manipulated the Mandalorians into attacking the Republic so as to weaken them ahead of their own planned invasion.

 

This is all quite possible, but bear in mind that we don't actually know, at least not yet, whether the Sith who manipulated the Mandalorians into war against the Republic were the true Sith or those few who might have remained from Exar Kun's ranks. Yes, I know Canderous mentions that the Sith came to the Mandalorians with an offer, but we don't know which Sith he refers to. And since the Mandalorians were allied with Exar Kun during the Great Sith War some 40 or so years before KotOR, it might seem just as likely that they were involved.

I actually think Revan already knew at this point. I think he found the Trayus Academy on Malachor V during the war and learned of the true Sith, then decided to use the planet's sinister nature to both destroy the Mandalorians and convert his own jedi followers to the Sith ways. Otherwise there would seem to be little reason for Revan to convert the jedi beyond having already fallen to the dark side.

That's how I see it to. At least to a point. Revan sacrificed himself by embracing the dark side and converting his jedi followers, so that he could build an army of followers to destroy the true Sith with. He then searched for the Star Forge to build an armada to send against them as well. But Revan didn't resist the dark side as much as he had hoped. He repeated Ulic's mistake and thought he could just wield the dark side as a weapon without being corrupted by it. In that he was mistaken. The dark side corrupted him, and when he looked at the Republic with new eyes, he no longer saw something to protect, but rather a feeble and vulnerable society that he could conquer for resources as a matter of course for his greater goal of destroying the true Sith and then rule the galaxy. He wanted to save the Republic. His intentions were good. But we all know what the road to Hell is paved with...

Im not sure if Revan really did fall, at conversations with Disciple, he say that Revan left numerous military factorys that he had chance to destroy, that Revan saw a limit of where the sith fleet come from. But then the "war" with the republic didnt go the way he wanted, and he got captured.
