How do KT's work?

How do KillTrackers actually work? Do they read incomming packets or something? Or do they just read the on screen text?

I'm no coder, but as far as I know, it reads the text on the screen.

i doubt that, filtering packets is actually a lot easier than reading stuff directly off the screen... but then again, i'm no expert
