Tags are all over the place

bolt_r_hand is in the left hand even though it's weighted to the rhang_tag bone. The bolt_l_hand stays nicely in it's place.

And the bolt_head_front is on the back of her head and the bolt_head_back in the front.

 

Does anybody know how to fix this?

 

I already deleted all the tags, merged them from Kyle's root.xsi and re-weighted them. Still they're all over the place
