Just ran through Narshadaa got 2 squads there(after Zez conv) and set both jrnl entries,went straight to Korriban and fought squad there and set 3rd jrn entry.

Problem is if the first 2 set there is no reason the 3rd shouldn't set(they all use the same global and scripts) unless there is a modification to your global jrn file causing the entry not to show up, in either case you should still get the HK torture scene aboard the EH.
