Sneak attacks... when do they kick in?

I'm wondering when sneak attacks kick in. I know if you are in stealth mode and attack somebody from behind you get the bonus. I also know that a stunned enemy gets the damage.

 

I was told that sneak attacks apply with a Guardian's Force Jump. Is this true? (cuz it would be awesome)

 

What about with ranged weapons?

 

What about stealthed attacks from the front?

 

What about non-stealthed attacks from behind?

1)Sneak attack always triggers from behind or when the enemy is incapacitated, I dunno about stealth though, never used a lot before

2)true, that's why I LOVE Scoundrel/guardian, who jumps from side to side in a battle all the time

3)true, within 30 feets, if the conditions in 1) applies

5)yes that IS a sneak attack, as said in 1)
