I think I understand you well enough. You've got the tumblers all lined up and got the scythe stuck in the gap. If you have gotten that far, then all you need to do is actually open the door, but you can't if Manny's head is still turned toward the scythe. Unless you say different, I'm going to assume you already know how to turn Manny's head since you've gotten this far into the game.
