Server Banning Questions

Few questions regarding banning.

 

How do you do it?

 

Is it purely IP based? Or can you ban based on other factors like hostmask or name?

 

If so, what is the syntax?

I find that I only get half of the ppl on the list.

Should is the key word there. The use of rcon status is an issue for several Q3 based games. Maybe around 2015, they'll finally fix it.
