Have you checked you have weighted the hand properly to the correct bones etc. ?

 

I had a similar problem with an inverted hand which turned out that I hadn't weighted it correctly

Sorry I misread the pic, I thought you had made a player model and the hand was detached from the body.

Software used to create/export that hilt?

 

If its 3d max, export your mesh to .3ds or .xsi and import it back into max if you see weird behavior that's not a good sign.

 

Is your hilt placed on the 0,0,0 axis? (well its pivot point should be and the mesh itself should line up with kyle's)
