Just to add to your numerous options Make doubly sure you're using/extracting the appearance.2da from the appropriate game. Check to make sure you're in the proper game node in KotOR Tool... Using app.2da from KotOR I in TSL spells problems.... and vice versa... This happened some time back, and it had a similiar effect to the problem you're describing...
