First, Hamlet is saying it to a nobleman (Polonius) so it's insinuating that his position is unwarranted. Second, fishmonger's daughters were generally considered to be... loose. He's basically calling Polonius a brothel-keeper. There, see... Shakespeare did it better - it's almost like Frasier. But just to join in: Thou spleeny puisny-tilted ingested-lump!
